Esperta Health is advancing in-home wound care through evidence-based practices, led by a Nurse Practitioner - Wound Care Team Leader. You will supervise wound care nurses, coordinate with interdisciplinary teams, and drive clinical outcomes in the assigned market.

The role emphasizes leadership, patient-centered care, and adherence to regulatory standards, with a focus on continuous quality improvement and staff development.

Qualifications

• Current, unrestricted NP licensure in state(s) of practice.
• WOCN or equivalent wound care certification preferred.
• Minimum 3–5 years of wound care clinical experience, preferably in home or post-acute settings.
• Demonstrated leadership experience in clinical settings or wound care programs.
• BLS certification or higher.
• Proficiency with electronic health records and digital wound care management tools.
• Excellent communication, organization, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Valid driver's license and reliable transportation for in-home visits.
• Strong commitment to patient-centered care and quality improvement.

As a Nurse Practitioner - Wound Care Team Leader, you will play a critical role in advancing our wound care program within the assigned market. This leadership position requires clinical expertise in wound care, strong team collaboration abilities, and a passion for improving patient outcomes. You will oversee clinical outcomes, support wound care nurses, and work closely with interdisciplinary teams to ensure excellence in in-home wound care delivery.

Responsibilities:

• Provide expert wound care assessments, treatment planning, and management in the home setting following evidence-based guidelines and company protocols.
• Lead and mentor wound care nurses within the assigned market to promote best practices and enhance clinical outcomes.
• Collaborate with interdisciplinary teams including physicians, case managers, and other healthcare professionals to coordinate patient-centered care.
• Monitor clinical quality, compliance, and performance metrics to ensure adherence to regulatory and company standards.
• Serve as a clinical resource and subject matter expert for wound care within the market
• Participate in continuous quality improvement activities, product evaluations, and new treatment protocol development.
• Engage with patients and caregivers to provide education, support, and ensure satisfaction with care delivery.
• Demonstrate leadership by fostering a culture of teamwork, accountability, and clinical excellence.

• Current, unrestricted Nurse Practitioner licensure in the state(s) of practice.
• Wound care certification such as WOCN or equivalent recognized wound care credential preferred
• Minimum 3-5 years of clinical experience with a strong background in wound care, preferably including in-home or post-acute care settings.
• Demonstrated leadership experience in clinical settings or wound care programs.
• BLS certification or higher.
• Proficiency with electronic health records and digital wound care management tools.
• Excellent communication, organization, and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
