Qualifications

• Active NP licensure in the relevant state(s).
• WOCN or equivalent wound care certification preferred.
• 3–5 years of clinical wound care experience, in-home or post-acute setting preferred.
• Proven leadership in clinical or wound care programs.

Responsibilities

• Provide expert wound care assessments and home-based treatment planning.
• Lead and mentor wound care nurses to improve outcomes.
• Collaborate with physicians, case managers, and health professionals for patient-centered care.
• Monitor quality, compliance, and performance metrics.
• Serve as clinical resource and SME for wound care in the market.
• Engage patients and caregivers with education and support.
