Job summary

Hackensack Meridian Health Inc. is seeking an Advanced Practice Nurse (APN) to deliver patient-centered care within a coordinated care model.

The role emphasizes leadership, direct clinical practice, and cross-disciplinary collaboration, with a focus on wound ostomy care. The APN will plan and implement care, work with the team to meet health and safety needs, and pursue ongoing clinical competency through education and practice.

Qualifications

• Must hold a Master’s or Doctorate degree from an NLN/AACN accredited Nurse Practitioner program.
• Current, unrestricted NJ State Professional Registered Nurse License.
• Current, unrestricted NJ Advanced Practice Nurse License.
• National board certification as a Nurse Practitioner from the AANP or ANCC.
• Valid prescriptive authority in New Jersey, including active CDS and DEA registrations.
• Current Basic Life Support (BLS) certification from the American Heart Association (AHA).

Job description

The APN works collaboratively with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Wound ostomy APN Preferred.

Required Qualifications

• Education:
  • Must hold a Master's or Doctorate degree from an NLN/AACN accredited Nurse Practitioner program.
• Licensure & Certifications:
  • Current, unrestricted NJ State Professional Registered Nurse License.
  • Current, unrestricted NJ Advanced Practice Nurse License.
  • National board certification as a Nurse Practitioner from the AANP or ANCC.
  • Valid prescriptive authority in New Jersey, including active CDS and DEA registrations.
  • Current Basic Life Support (BLS) certification from the American Heart Association (AHA).
• Knowledge, Skills & Abilities:
  • Proven experience in a relevant patient care setting.
  • A collaborative practice agreement with a staff physician must be in place.
  • Active National Provider Identifier (NPI).
  • Excellent written and verbal communication skills.
  • Proficiency with computer platforms, including Microsoft Office and/or Google Suite.
  • A demonstrated commitment to maintaining clinical competencies through ongoing education and practice.
• Certifications:
  • AHA Current Advanced Cardiac Life Support (ACLS) certification.
