Qualifications

Board Certified as a Family Nurse Practitioner
Active Family Nurse Practitioner license in Louisiana
Consistently exhibits professionalism, integrity, accountability, confidentiality, care and compassion
Willingness to work in a revolutionary environment with last-minute problem solving
Technologically savvy with charting and secure messaging apps (HIPAA)
Strong written and verbal communication
Ability to work independently or in a team
Pass national background check and valid clinical license search

Responsibilities

Review your case load and prepare your technology to service patients.
Navigate your schedule with Sprinters to visit each patient’s home.
Build warm, welcoming connections with a diverse range of patients.
Conduct health risk assessments, medication reviews, cognitive screenings and provide health education.
Collaborate with Sprinters to evaluate vital signs, arrange blood draws, and address patient needs.
Provide comprehensive care coordination including preventive care interventions and referrals.
