Job summary

Oak Street Health seeks a Nurse Practitioner to deliver value-based primary care to Medicare patients in our outpatient clinics. You will work autonomously with physicians on a collaborative care team to provide longitudinal care and build trusted patient relationships.

Qualifications

• MD/NP-level advanced clinical qualification in primary care for adults.
• National certification in one of the listed NP specialties.
• Active, unrestricted NP license in the state of practice.
• Active DEA license and US work authorization.

Responsibilities

• Provide effective and equitable value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Perform annual wellness visits, health risk assessments, and comprehensive documentation.
• Coordinate care with other providers, testing facilities, and care managers.
• Lead population health initiatives and assist with outreach and education of patients.
