Job summary
Oak Street Health seeks a Nurse Practitioner to provide value-based primary care for adults on Medicare in outpatient clinics. You will work within a collaborative care team with physicians, PAs, nurses, and care managers to ensure the best patient outcomes and strong patient relationships.
The role emphasizes comprehensive wellness visits, care coordination, patient education, and participation in marketing activities.
Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- National certification in at least one of the specified specialties (Family NP, Adult-Gerontology PCP NP, Adult NP, Gerontological NP).
- Active, non-probationary state NP license
- Active DEA license
- US Work Authorization
Responsibilities
- Provision of exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer Annual wellness visits and health risk assessments with thorough documentation.
- Care coordination with other providers, specialists, testing facilities, and agencies.
- Population health leadership in coordination with the Care Team.
- Assisting the care team with phone triage and outreach.
- Educating patients on health conditions and care plans.
- Participating in Oak Street Health promotional activities.
- Conducting home visits as needed.
