Job summary

Qualifications
- MSN, DNP, or NP certificate program graduate.
- National certification in at least one listed specialty (FNP, AGPCNP, ANP, GNP).
- Active, non-probationary NP license in the appropriate state.
- Active DEA license.
- US work authorization.
Responsibilities
- Provide exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer annual wellness visits and health risk assessments with thorough documentation.
- Coordinate care with other providers, specialists, facilities and agencies.
- Lead population health initiatives in coordination with the care team.
