Job summary

Oak Street Health is seeking a Nurse Practitioner to join our outpatient care team in Jennings, MO. The NP will provide value-based primary care to adults on Medicare, conduct visits, wellness checks, and coordinate care with physicians and specialists.

The role reports to the Center Medical Director and requires NP licensure and national certification. Compensation ranges from $129,000 to $148,000 annually, with a comprehensive benefits package and full-time hours.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in at least one of: Family Nurse Practitioner, Adult-Gerontology Primary Care NP, Adult NP, Gerontological NP.
• Active, non-probationary state NP license.
• Active DEA license.
• US Work Authorization.

Responsibilities

• Provide exceptional primary care on a collaborative care team.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assist the care team with phone triage and outreach.
• Educate patients on health conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties, as assigned.
