Job summary

Your Health in South Carolina is seeking a qualified Nurse Practitioner to provide in-home patient care across Beaufort area. As a Provider, you will evaluate high-risk patients, coordinate care plans, and collaborate with an interdisciplinary team to improve outcomes.

This full-time role offers competitive pay and a monthly bonus, with shifts including 12-hour or 8-hour options. Travel between offices and facilities is required, and HIPAA compliance is expected.

Qualifications

• Licensed NP with SC license in good standing.
• Experience in home health, clinic, or telehealth preferred.
• Strong communication and teamwork skills.
• Ability to evaluate high-risk patients and create care plans.
• Willingness to travel between offices and facilities.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity and optimal outcomes.
• Communicate with case managers, coordinators, and others to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss progress and plans.
• Document care management activities accurately for regulatory compliance.
• Educate patients and families on treatment plans and self-management.
