Job summary

Your Health in South Carolina seeks a licensed Nurse Practitioner (Psych) to deliver telehealth care to patients across the state, with semi‑monthly facility visits in Columbia. The role is full‑time with a monthly bonus opportunity and collaboration within a multi‑disciplinary team.

Responsibilities include evaluating high‑risk patients, coordinating care plans, and providing education and resources for self‑management, while ensuring HIPAA compliance across virtual and in‑person settings.

Qualifications

• NP/PA/Physician with SC license in good standing.
• Experience in home health, clinic, facility, telehealth, or specialty care is preferred.
• Strong communication and collaboration within an interdisciplinary team.
• Ability to evaluate high‑risk patients and develop care plans.

Responsibilities

• Evaluate high‑risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity and outcomes.
• Communicate regularly with case managers, coordinators, social workers, specialists, pharmacists, and health coaches.
• Participate in team meetings to discuss patient progress and treatment plans.
• Foster collaboration to support seamless patient management across settings (homes, facilities, telehealth, clinics).
• Meet efficiency expectations while providing high‑quality care.
• Document care activities accurately to support care plans and compliance.
• Provide patients with education and resources for conditions and self‑management.
