Qualifications

• Master's degree in nursing from an accredited program.
• Current Nurse Practitioner certification in Family Medicine.
• Valid state nursing license and certification to practice as a Nurse Practitioner.
• Be able to work independently and with a multidisciplinary team in a quality-based care environment.
• Exhibit critical thinking, time management, and organizational skills.

Responsibilities

• Deliver comprehensive clinical assessments and develop treatment plans.
• Perform telehealth Transitional Care Management visits and update problem lists.
• Collaborate with multidisciplinary team to provide holistic care.
• Educate patients and families to empower informed health decisions.
• Support quality scores and population health initiatives.
