South Central Regional Medical Center in Laurel, MS is seeking a Nurse Practitioner for our Population Health/ACO program. You will conduct clinical assessments, develop care plans, and perform telehealth TCM visits, focusing on Medicare populations.

The role requires strong independence, critical thinking, and ability to collaborate with a multidisciplinary team to improve quality scores and patient outcomes. PRN/on-site position.

Qualifications

• Master's degree in nursing from an accredited program.
• Current Nurse Practitioner certification in Family Medicine.
• Valid state nursing license and certification to practice as a Nurse Practitioner.
• Be able to work independently and with a multidisciplinary team as well as function/work effectively within an ever-changing quality-based care environment.
• Exhibit critical thinking skills, great time management, and organizational skills as well as quick learner

Responsabilités

• Conduct comprehensive clinical assessments and develop treatment plans for Medicare and Medicare Advantage populations.
• Perform telehealth TCM visits and update problem lists to ensure patient safety.
• Collaborate with a multidisciplinary team to deliver holistic care and patient education.
• Develop and implement individualized care plans with patients and healthcare teams.
