Job summary

Purposelink Recruitment seeks a Nurse Practitioner to deliver value-based, telehealth care for medically complex patients in a growing program across Texas and New Mexico.

This contract role focuses on home-based care, prevention, and outcomes, with collaboration across an interdisciplinary team. Spanish-speaking candidates are encouraged, and licensure flexibility is offered where possible.

Qualifications

  • Master's degree or higher required.
  • Telemedicine care experience of 2+ years.
  • 3+ years of recent primary care experience.
  • 1+ year of longitudinal patient care experience.
  • 1+ year of value-based care model experience.
  • Spanish language proficiency preferred, not required.
  • Live in Texas or New Mexico.
  • Active NP license or eligible for NM licensure.

Responsibilities

  • Provide clinical care and ongoing management via telehealth and remote monitoring.
  • Manage medically complex Medicaid and Dual Eligible populations.
  • Develop and execute individualized care plans with physicians and teams.
  • Collaborate with patients, families, caregivers, specialists and health plans.
  • Identify changes in health and intervene to prevent avoidable ED visits.
  • Support patients through hospital admissions, discharges, and transitions home.
  • Conduct advance care planning and goals-of-care discussions.
  • Participate in triage and after-hours support models.
  • Document to support HCC coding, risk adjustment, STAR/HEDIS measures, and close care gaps.
  • Contribute to clinical best practices and evolving care model.

Required Qualifications
  • Master's degree or higher
  • 2+ year of Telemedicine Care experience
  • 3+ years of recent Primary Care experience
  • 1+ year of longitudinal patient care experience
  • 1+ year of experience working in a value-Based care model
  • Full proficiency in speaking, reading, and writing Spanish- Preferred not required.
  • Must live in Texas or New Mexico
  • Active Nurse Practitioner license (NM or TX candidates able to obtain NM licensure)
Overview

This contract role offers an opportunity for an experienced Nurse Practitioner to help redefine care for medically complex patients in a growing, innovative value-Based care organization expanding into New Mexico. This is not a traditional clinic position - it's a chance to deliver proactive, home-centered care through telehealth, remote monitoring, and interdisciplinary collaboration.

The ideal NP has strong clinical acumen, experience managing complex adult populations, and a passion for improving outcomes through a value-Based care model. Candidates with startup experience or comfort working in evolving care environments are highly encouraged to apply.

Key Responsibilities

  • Work closely with patients, families, caregivers, specialists, and health plan partners
  • Identify changes in patient health and intervene early to prevent avoidable ED visits and hospitalizations
  • Support patients through hospital admissions, discharges, and transitions back home
  • Conduct advance care planning and goals-of-care discussions
  • Participate in shared clinical triage and after-hours support models
  • Ensure accurate documentation supporting HCC coding, risk adjustment, STAR/HEDIS quality measures, and closure of care gaps
  • Contribute to clinical best practices and help shape a growing care model

Pay Transparency Compensation varies based on location, experience, and performance. Some sites require disclosure of pay and benefits. This role may also be eligible for bonuses or incentives depending on business needs. Employees may receive health insurance, optional dental and vision coverage, life and disability insurance, retirement savings plans, paid holidays, and PTO or sick time. The estimated hourly rate for this position is $135k.
