Job summary

Johns Hopkins Community Physicians is seeking a Nurse Practitioner to join the White Marsh SHARP team. The SHARP model supports high-acuity patients with a small, dedicated panel co-managed by several PCPs.

You will provide enhanced primary care, enjoy regular hours with occasional night call, and have access to strong interdisciplinary support, a dedicated medical assistant, and a competitive benefits package.

Qualifications

• Maryland CRNP license required.
• CPR certification required.
• 3 years' experience preferred.

Responsibilities

• Provide high-quality primary care within the SHARP model.
• Co-manage a small panel of high-risk adult patients with 3–5 PCPs.
• Deliver extended visits and participate in a collaborative care approach.

Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner to join our White Marsh practice new SHARP model. SHARP (small high acuity/risk panel) is a Nurse Practitioner led small panel primary care program. Johns Hopkins SHARP providers co-manage a small panel of high-risk adult patients in a primary care practice, enhancing standard PCP care. The SHARP Nurse Practitioner collaborates with 3-5 established PCPs to provide enhanced, high touch, independent and collaborative primary care to a select group of high-risk patients. SHARP NPenjoy extended visits and significantly lower visit volumes than seen in standard primary care practices.
