Job summary

Johns Hopkins Community Physicians seeks a Nurse Practitioner to join our Home-Based Primary Care Program in Baltimore, Howard County, and Anne Arundel County in Maryland. The NP will evaluate and treat home-bound, medically complex older adults, coordinating care with the Medical Director and other team members.

Three to five years NP inpatient or outpatient practice preferred; Maryland licensure and CPR certification required.

Qualifications

• Must have Maryland NP licensure and CPR certification.
• 3 years’ experience in primary care or acute/urgent care preferred.
• Palliative or geriatric certification/experience preferred.
• Must have a working vehicle to travel to patient homes.

Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner to join our Home-Based Primary Care Program in Baltimore, Howard County, and Anne Arundel County in Maryland. This growing program focuses on high needs geriatric home-limited patients and provides high-touch, comprehensive primary care. Collaborating with the Medical Director and other team members, the Nurse Practitioner (NP) will provide medical care for a small panel of home bound and medically complex older adults. The NP will be responsible for evaluating and treating patients receiving primary care at home, coordinating their care with other team members and specialists, and participating in weekly inter-disciplinary team meetings. Three to five years NP inpatient or outpatient practice preferred. General medical experience with geriatric, medically complex and high acuity patients desired

• Part time, 20 hours clincial and 4 hours admin
• NP Licensure in the State of Maryland required.
• Must have a working vehicle to travel to patient's homes.
• CPR Certification required.
• 3 years’ experience preferred in primary care or acute/urgent care.
• Palliative and/or geriatric certification/experience preferred.

Johns Hopkins Community Physicians (JHCP) is looking for a Nurse Practitioner to join our Remington practice for the remaining .5 FTE. Johns Hopkins Community Physicians serves Maryland, Virginia and Washington DC with over 40 locations.

• Part -Time- 20 hour clinical and 4 hours admin
• Admin time may be remote
• Night call once per month
