Job summary

Johns Hopkins University in Baltimore's Department of Medicine, Division of General Internal Medicine, seeks a Nurse Practitioner for outpatient clinic to co-manage a small panel of high-risk adult patients in primary care. The NP will collaborate with resident and faculty PCPs at Johns Hopkins Outpatient Center Medical Clinic to provide enhanced, high-touch, independent and collaborative care.

The role features extended visit times and lower visit volumes to develop meaningful relationships,

Qualifications

• Master's degree in Nursing and NP certification.
• MD RN license with NP certification in Maryland.
• Current CPR certification.
• Maryland CDS and DEA licensure.

Responsibilities

• Work schedule includes 40 hours per week with clinic and administrative sessions.
• Assist with comprehensive care of high-risk patients through chart review, history, and exams.
• Coordinate care with interdisciplinary teams and manage telehealth or in-person visits.
• Provide post-discharge coordination to reduce readmission and ER use.
• Develop specific care plans with meds, follow-ups and sick-day plans.
• Counsel patients and families on disease processes and outcomes.
• Refer to specialists as needed and document in EMR for continuity.
• Participate in weekly high-risk rounds and provide updates to leadership.
• Collaborate with leadership to identify high-need patients and improve workflow.
