Job summary

The Johns Hopkins Outpatient Center Medical Clinic in Baltimore, MD seeks a Nurse Practitioner to co-manage a small panel of high-risk adults in a primary care setting. Qualifications

• Master's Degree in Nursing and NP certification.
• Must be licensed as NP in Maryland; current CPR certification required.
• Maryland CDS and DEA licensure required; JHH credentialing prior to start.

Responsibilities

• Co-manage a small panel of high-risk adult patients in outpatient clinic.
• Collaborate with PCPs and interdisciplinary teams to align care with patient goals.
• Document visits and follow-ups in EMR and coordinate post-discharge follow-up.
• Provide telemedicine or in-person visits with extended visit times.
• Coordinate care with other specialists and provide patient/caregiver education.
