Job summary

Conviva Senior Primary Care in Texas invites a Nurse Practitioner or Physician Assistant to join our on-site Jourdanton clinic. You’ll join a collaborative, multidisciplinary team delivering value-based care to seniors, with a focus on personalized, high-quality outcomes.

You’ll report to the Associate Medical Director, work full-time (40 hours/week), and participate in on-call rotation while contributing to growth plans at your center. Competitive benefits and bonus eligibility apply.

Qualifications

• Master’s in Nursing or completed PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening required if selected for this role.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately with support from quality-based coders.
• Participate in on-call rotation and support center growth plans.
• Ensure value-based care delivery.
