Job summary

Position

Optum is seeking a Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Salem/Eugene, OR. Optum is a clinician-led care organization that creates a seamless health journey for patients across the care continuum.

Position Highlights

In Optum's Senior Living Program, clinicians are supported to reach their full potential. The model focuses on assisted living and other community-based settings, blending palliative care with internal medicine. NPs and PAs provide care to our highest-risk patients as part of an interdisciplinary team.

We offer extensive training in a non-fee-for-service model focused on quality patient outcomes. Expect approximately 4–6 patient visits per day, allowing time to invest in patient well-being and relationship-building.

Responsibilities
  • Conduct comprehensive assessments
  • Manage patients through acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
  • Manage patients during short-term sub-acute rehabilitation episodes
  • Ensure accurate and complete documentation, including ICD-10
  • Communicate and collaborate with the interdisciplinary care team
  • Conduct advanced illness and advanced care planning conversations
  • Provide patients and caregivers with counseling and education
  • Perform comprehensive assessments and document findings in a concise, CMS-compliant manner
  • Facilitate agreement and implementation of the care plan with facility staff, families, primary and specialty care clinicians
  • Coordinate care as patients transition through different levels of care and care settings
  • Review orders and interventions for appropriateness and identify the most effective plan of care
  • Function independently and responsibly with minimal supervision

Required Qualifications
  • For Nurse Practitioners:
    • Graduate of an accredited Master of Science in Nursing or Doctor of Nursing Practice program
    • Active and unrestricted NP license in the state of residence or ability to obtain by start date
    • Board certified through AANP or ANCC, with certification in one of the following: Family NP, Adult NP, Gerontology NP, Adult-Gerontology Acute Care NP, or Adult-Gerontology Primary Care NP
  • For Physician Assistants:
    • Graduate of an accredited Physician Assistant program
    • Active and unrestricted PA license in the state of residence or ability to obtain by start date
    • Board certified by NCCPA
    • Active DEA license/prescriptive authority or ability to obtain post-hire (where allowed by state)
    • Ability to gain a collaborative practice agreement, if applicable in your state
    • Ability to complete physical requirements of the job, including lifting a 30-pound bag, stairs, varied dwelling conditions, and performing physical assessment
    • Access to reliable transportation
  • Other requirements as applicable to the role and state regulations
Preferred Qualifications
  • New graduates are encouraged to apply
  • 1+ years of clinical experience in senior living, geriatric medicine, or home care
  • Experience addressing healthcare needs of patients with complex behavioral, social, or functional needs
  • Understanding of geriatrics, chronic illness, and advanced illness/end-of-life discussions
  • Proficient computer skills and ability to document in electronic medical records

Note: Employees must be in active regular status and remain in the role for a minimum of 12 months from hire/rehire/transfer. If an employee leaves Home and Community, student loan repayments cease. The employee must remain in an Advanced Practice Clinician or Physician role within Home and Community for 36 months to receive the full loan repayment benefit.
