Qualifications

• Nurse Practitioner certification from a national board.
• MSN or equivalent master's in nursing.
• Board certification in ANP, FNP, ACNP, AGACNP, or Gerontology/Adult-Primary care.
• Active/unrestricted license in the practice state or ability to obtain by start date.

Responsibilities

• Deliver primary care to assigned patients in home/ SNF settings.
• Perform comprehensive assessments and document findings per CMS rules.
• Review medications and reconcile post-hospitalization plans.
• Coordinate care with physicians, facilities, families, and other caregivers.
• Travel between care sites as required by the role.
