Qualifications

• Actively Licensed to practice in the state of employment.
• Current DEA License (required).
• ANCC/AANP Certification required.

Responsibilities

• Collaborate with therapists, nurses and other healthcare professionals to develop comprehensive rehab treatment plans.
• Conduct patient assessments including histories, exams and diagnostics to set rehabilitation goals.
• Monitor progress and adjust treatment plans to improve function and independence.
• Document evaluations, plans, progress notes and discharge summaries in EMR.
• Educate patients and families about rehabilitation process, outcomes and post-discharge care.
