This field-based role emphasizes preventive care, education, and care coordination with PCPs to close gaps and improve outcomes while supporting work-life balance with flexible scheduling between 8am-8pm across a 7-day week.

Qualifikationen

• Active certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted license to practice in the current state without disciplinary actions.
• At least 1 year of clinical experience; home health preferred.
• Valid driver's license and ability to navigate varied home environments.

Aufgaben

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent situations and intervene appropriately.
• Educate members on diseases, medications, and adherence; address social determinants of health.
