Job summary

Suncrest Hospice in Quakertown, PA is seeking a compassionate Nurse Practitioner to perform physical exams and preventive care within hospice guidelines. You will collaborate with physicians and the inter-disciplinary team to deliver high-quality patient-centered care.

The role requires NP certification and state licensure, a commitment to driving to patient homes, and the ability to coordinate with staff to develop comprehensive care plans.

Qualifications

• Requires completion of accredited Nurse Practitioner program.
• State licensure in good standing.
• Must be willing to drive with reliable transportation, valid driver's license, and auto insurance.

Responsibilities

• Perform physical examinations and preventive health measures.
• Record physical findings and formulate care plans.
• Discuss cases with physician and other health professionals to prepare comprehensive patient care plan.
• Prescribe or recommend drugs or other forms of treatment.
• Monitor and maintain progress of patients and report progress to supervising physician.
• Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT.

Details

The Nurse Practitioner performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician.

• Records physical findings, and formulates plan and prognosis, based on patient's condition.
• Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
