Job summary

Brighton Hospice is seeking a Nurse Practitioner in Indiana to provide comprehensive hospice care within prescribed guidelines. You will perform physical examinations, assess patient conditions, and develop care plans with physicians.

You will prescribe treatments, monitor progress, and coordinate with the interdisciplinary team to ensure high-quality patient care. A valid NP license and driving ability are required.

Qualifications

• Must have completed an accredited Nurse Practitioner program.
• State licensure in good standing.
• Willingness to drive with reliable transportation and valid driver’s license.

Responsibilities

• Records physical findings and formulates plan and prognosis with physician guidance.
• Discusses cases with physician and other health professionals to prepare patient care plans.
• Prescribes or recommends drugs or other treatments as appropriate.
• Monitors patient progress and reports to supervising physician.
• Works collaboratively with hospice staff and volunteers as part of the IDT.

Details

The Nurse Practitioner performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician.

• Records physical findings, and formulates plan and prognosis, based on patient's condition.
• Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
• Prescribes or recommends drugs or other forms of treatment.
• Monitors and maintains progress of patients and reports progress to supervising physician.
• Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT

Qualifications

• While performing the duties of this position, the employee is regularly required to assist in lifting and transferring patients as required.
• Specific vision abilities required by this job include: close vision, distance vision and ability to adjust focus.
• Requires completion of accredited Nurse Practitioner program.
