Job summary

Premise Health seeks a Nurse Practitioner for our Centralized Staffing PRN Pool in San Antonio, TX. This role involves providing compassionate care to employees and dependents across multiple on-site Health Centers, with travel between four local locations.

Expect no nights or weekends and 3–4 days per week availability. You must maintain NP licenses, national certification, and prescriptive authority as applicable.

Qualifications

• Licensed Nurse Practitioner with current state authority.
• National certifcation (ANCC or AANP) required.
• DEA license and prescriptive authority as applicable.
• At least 3 years of ambulatory/occupational health or family practice experience.

Responsibilities

• Conduct physical exams and preventive health care per guidelines.
• Perform injections, suturing, wound care, and infection management.
• Develop patient care plans and record progress for continuity.
• Educate patients on treatment plans and health maintenance.
• Order and interpret diagnostic tests to assess conditions.
• Collaborate with physicians and health care teams for care.
• Prescribe or recommend treatments and therapies.
• Refer patients to specialists when needed.
• Maintain accurate patient health records and documentation.
• Educate and coach nursing staff on best practices.
• Perform other duties as assigned.

As a Nurse Practitioner in our Centralized Staffing PRN Pool, you'll provide care to client employees and their dependents in our Health Centers located in San Antonio, TX. Looking for availability 3-4 days per week with the ability to travel to four locations in the San Antonio area. No nights or weekends!

What You’ll Do

• Conducts physical exams and preventive health measures per guidelines
• Performs procedures such as injections, suturing, wound care, and infection management
• Develops patient care plans, records progress, and ensures continuity of care
• Educates patients on treatment plans, health maintenance, and development
• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies
• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You’ll Bring

• Maintains state RN licenses/certificates to practice
• Maintains Nurse Practitioner licenses/certificates to practice
• Maintains State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands‑on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
