Job summary

Premise Health Inc. in San Antonio, TX is seeking a Nurse Practitioner to join our Centralized Staffing PRN Pool. Availability is 3-4 days per week with travel to four San Antonio locations. No nights or weekends!

Responsibilities include conducting exams, procedures, care planning, testing, education, and collaboration with physicians. Requires NP licensure, DEA, national certs, MSN, BLS/ACLS and 3+ years in ambulatory care.

Qualifications

• Maintains state RN licenses/certificates to practice.
• Maintains Nurse Practitioner licenses/certificates to practice.
• Maintains State Prescriptive Authority (if applicable).
• Federal DEA license (matching each state of practice as applicable).
• State Narcotic/Controlled Substance license if required in your state.
• Current national certification (ANCC or AANP required).
• Master’s Degree in Science of Nursing.
• Current certification with hands‑on training in BLS/ACLS (as applicable).
• At least three years of clinical NP experience in ambulatory care, occupational health, family practice or ED.

• Educates patients on treatment plans and health maintenance.
• Orders and interprets diagnostic tests to assess clinical conditions.

• Prescribes or recommends treatments, including medications and therapies.
• References patients to specialists or physicians as needed.
• Maintains patient health records with up‑to‑date documentation.
• Educates and coaches nursing staff on best practices.
• Performs other duties as assigned.

What You’ll Do

• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records

What You’ll Bring

• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands‑on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
