Job summary

Premise Health Inc. is seeking a Part-Time Nurse Practitioner to provide primary care for employees and dependents aged 3 and up at our Hanover, PA health center. The provider will work in a collaborative setting with a focus on preventive care and patient relationships.

Schedule: Tue 11:00 AM–7:00 PM, Thu 11:00 AM–4:00 PM, Fri 8:00 AM–3:00 PM. Requires active NP license, prescriptive authority, national certification, and ambulatory care experience.

Qualifications

• Active NP license with state prescriptive authority
• National certification (ANCC or AANP) required
• Master's in Nursing; transcripts if MS certificate not Nursing
• BLS/ACLS certification as required by contract scope

Responsibilities

• Conduct physical exams and preventive health measures per guidelines
• Perform injections, suturing, wound care, and infection management
• Develop patient care plans, record progress, ensure continuity of care
• Educate patients on treatment plans, health maintenance, and lifestyle changes
• Order and interpret diagnostic tests to assess conditions
• Collaborate with physicians and health-care teams for comprehensive care
• Prescribe or recommend treatments and refer to specialists as needed

As a Part-Time Nurse Practitioner, you’ll provide primary care to client employees and their dependents ages 3 and up at our Health Center in Hanover, PA. This is a great opportunity for an NP with a primary care background who enjoys building relationships with patients and providing personalized, convenient care in a collaborative health center setting. The position has a schedule of Tuesday 11:00 AM-7:00 PM, Thursday 11:00 AM-4:00 PM, and Friday 8:00 AM-3:00 PM.

What You'll Do

• Conducts physical exams and preventive health measures per guidelines


• Performs procedures such as injections, suturing, wound care, and infection management


• Develops patient care plans, records progress, and ensures continuity of care


• Educates patients on treatment plans, health maintenance, and development


• Orders and interprets diagnostic tests to assess clinical conditions


• Collaborates with physicians and healthcare teams for comprehensive care


• Prescribes or recommends treatments, including medications and therapies


• Refers patients to specialists or physicians as needed


• Maintains patient health records ensure accurate and up-to-date documentation and records


• Educates and coaches nursing staff on best practices


• Performs other duties as assigned

What You'll Bring

• Maintains state RN licenses/certificates to practice


• Maintains Nurse Practitioner licenses/certificates to practice


• Maintains State Prescriptive Authority (if applicable)


• Federal DEA license (matching each state of practice as applicable)


• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)


• Current national certification (ANCC or AANP required)


• Master's Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)


• Current certification with hands‑on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services


• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting
