Qualifications

• Graduate of an Accredited Nurse Practitioner Program (Masters level preferred).
• Licensed by the Commonwealth of Massachusetts board of registration in Nursing.
• National Board Certification.
• Mass DPH Controlled Substances Registration or ability to obtain post-hire.
• DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification or equivalent professional certification.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of care for a panel of patients.
• Examination, diagnosis and treatment in a primary care setting; supervise care plans and referrals.
• Monitor and manage patients with acute and chronic illnesses; adjust treatment plans as needed.
• Partner with patients and families to manage care plans.
• Provide consultation regarding hospitalized patients in the panel.
• Provide direction and support to staff to ensure department effectiveness.
