Job summary

Imagine360 is seeking a Nurse Practitioner to join the team at on-site primary care clinics. The NP will deliver office-based primary care for patients of all ages, with chronic disease management and acute episodic care emphasis.

Responsibilities include exams, diagnostic testing, care coordination, and prescribing treatments while maintaining HIPAA-compliant documentation and strong patient communication.

Qualifications

• Degree from accredited nursing program.
• Active Florida APRN license with Nurse Practitioner certification and ongoing education.
• Current BLS certification and basic computer skills.
• Minimum of three years of clinical experience.

Responsibilities

• Deliver office-based primary care for patients of all ages.
• Manage chronic diseases with emphasis on wellness and acute care as needed.
• Coordinate care with miCare team and other specialists.
• Perform exams, order and interpret tests, document findings and plan.
• Prescribe appropriate treatments and follow up with patients.
