Job summary

One to One Health is seeking a Board Certified Nurse Practitioner or Certified Physician Assistant to join our clinic team. You will operate under supervising physicians’ protocols to evaluate, treat, and manage acute and chronic health issues, while performing wellness physicals and health screenings for employees.

Responsibilities include documenting in the EMR, coordinating referrals, and educating patients on care plans.

Qualifications

Active NP/PA license with board certification and 1 year of experience in family medicine, internal medicine, or urgent care.
DEA license preferred; BLS certification desired.

Responsibilities

Perform pre-employment and annual wellness physicals.
Manage acute and chronic health problems using evidence-based protocols under supervising physicians’ protocols.
Provide health screenings and disease prevention programs.
Evaluate and treat patients for injury and illness.
Enter patient data into the EMR system accurately and promptly.
Collaborate with multidisciplinary team members and refer as needed.
Provide patient education, health coaching, and advocacy.
Order and analyze diagnostic tests and monitor intervention effectiveness.
