Oak Street Health in Chandler, AZ is seeking a Nurse Practitioner to deliver effective value-based primary care to adults on Medicare. You will work within a collaborative care team in our outpatient clinic, providing comprehensive assessments and coordinated care.

Qualifications

• MSN or DNP/NP cert required.
• National NP certification in one of the listed specialties.
• Active, non-probationary NP license.
• Active DEA license.
• US work authorization.

Responsibilities

• Provide effective, value-based primary care to adults on Medicare.
• See patients independently and collaborate with physicians as needed.
• Deliver longitudinal care and build patient relationships.
• Coordinate care with physicians, specialists, and care managers.
• Participate in home visits and outreach as needed.
• Support population health initiatives and care team activities.
