Job summary

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in at least one of the listed NP specialties
• Active, non-probationary state NP license and DEA license; US work authorization

Responsibilities

• Provide exceptional primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and HRAs with thorough documentation.
• Coordinate care with other providers and facilities.
• Lead population health initiatives with the care team.
• Assist with triage and outreach by phone.
• Educate patients on conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
