Job summary

Oak Street Health in Winston-Salem, NC is seeking a Nurse Practitioner to deliver value-based primary care to adults on Medicare within outpatient clinics. You will work on a collaborative care team to provide independent patient care, build strong patient relationships, and participate in care coordination and population health initiatives.

The role emphasizes comprehensive wellness visits, documentation accuracy, and teamwork with physicians, PAs, nurses, and care managers.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization
• National certification in at least one of: Family Nurse Practitioner; Adult- Gerontology Primary Care NP; Adult NP; Gerontological NP

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and health risk assessments.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on health conditions and care plans.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
