Job summary

Oak Street Health in New York is seeking a Nurse Practitioner to provide value-based primary care to adults on Medicare within an outpatient setting, collaborating with a multidisciplinary team to keep patients healthy and out of the hospital.

You will lead annual wellness visits, coordinate care with physicians and specialists, and document care plans comprehensively as part of a patient-centered care model.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• Active, non-probationary NP license in the US; Active DEA license
• US work authorization; National NP certifications listed in Required Qualifications

Responsibilities

• Provide exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists, testing facilities, and agencies.
• Lead population health efforts in coordination with the care team; ensure appropriate screenings.
