Job summary

Oak Street Health in Fayetteville, NC is seeking a Nurse Practitioner to provide effective, equitable, value-based primary care to adults on Medicare within our outpatient clinics. NPs practice independently while collaborating with physicians as needed, delivering longitudinal care and building trusting relationships with patients.

The role supports a collaborative care team and involves care coordination, patient education, and participation in promotion activities.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate is required.
• Active, non-probationary state NP license and DEA license are required.
• US Work Authorization is required.

Responsibilities

• Provide exceptional primary care to adults on Medicare.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists and testing facilities.
• Lead population health initiatives and support the care team with outreach and education.
