Nebraska Methodist Health System is seeking a Nurse Practitioner to provide primary health care within our post-acute SNF settings. You will perform assessments, diagnose conditions, develop treatment plans, and collaborate with physicians to ensure high-quality patient outcomes.

The role requires MSN or DNP, current NP certification, and an active RN license, with a schedule Monday–Friday 8a-5p and rotating on-call nights, weekends, and holidays.

Qualificações

• MSN or DNP required for Nurse Practitioner licensure.
• Two years clinical experience preferred.
• Current RN license and NP certification required.

Responsabilidades

• Provide primary health care using assessment and diagnosis processes.
• Collaborate with physicians and other disciplines to coordinate patient care.
• Perform telephone triage and schedule follow-up appointments as needed.
• Document patient care and maintain complete records.
• Apply evidence-based practices and patient education to improve outcomes.
