Job summary

Upperline Health in Fontana, CA seeks a Nurse Practitioner to deliver clinic-based care in collaboration with the local physician team. APPs develop care plans, diagnose, and treat in clinic, home, or telehealth settings, following the APP’s scope and patient goals.

Ideal candidates will have Primary Care/Adult‑Gerontology experience, DEA registration, BLS, and a passion for compassionate, team‑oriented care in a fast‑growing, value‑based environment.

Qualifications

• Master’s degree in nursing is required.
• Board certification as NP in relevant specialty.
• DEA registration or eligibility.
• Experience in value‑based or home‑based care preferred.
• BLS certification required.

Responsibilities

• Develop and implement clinical plans of care for adults with chronic conditions.
• Gather patient history, exams, and diagnostics in home or clinic.
• Develop and implement treatment plans aligned with patient goals.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Educate patients and families about medications, diet, and crisis prevention.
• Prescribe medications as permitted by the state Board of Nursing.
• Maintain patient medical records per state regulations and policy.
• Participate in weekly team meetings and ongoing education sessions.
• Provide telephonic support for escalated calls from triage nurse.
