Job summary

Community Health Partnership of Illinois (CHP) is seeking a compassionate Nurse Practitioner to join our primary care team. The NP will deliver patient-centered care across the lifespan in a collaborative, multi-disciplinary setting.

The role requires an NP degree, Illinois APRN license, national certification, and BLS/CPR. Strong assessment, communication, and teamwork skills are essential, with bilingual abilities considered a plus.

Qualifications

• Master's or Doctoral degree from an accredited NP program.
• Current and unrestricted Illinois APRN license or eligibility.
• Current national NP certification from a recognized certifying body.
• Current RN license in Illinois, as required.
• Current DEA registration or eligibility to obtain registration if prescribing controlled substances.
• Current BLS/CPR certification.
• Strong clinical assessment, diagnostic, and patient-care skills.
• Excellent communication, organizational, and interpersonal skills.
• Bilingual skills are a plus.

Responsibilities

• Provide comprehensive primary care services to patients across the lifespan.
• Conduct physical examinations, health assessments, and preventive care.
• Diagnose and manage acute and chronic health conditions.
• Order, perform, and interpret diagnostic and laboratory tests.
• Develop and implement individualized treatment plans and care
• Prescribe medications within scope of practice.
• Educate patients on diagnoses, medications, and healthy lifestyles.
• Maintain accurate documentation in the EHR.
• Collaborate with physicians and healthcare team to coordinate care.
• Make referrals to specialists and community resources as needed.
