Job summary

Community Health Partnership of Illinois (CHP) in Champaign, IL seeks a Family Nurse Practitioner to provide high-quality primary care to patients across the lifespan and support CHP’s commitment to accessible, culturally responsive healthcare.

The FNP will perform exams, diagnose and treat common illnesses, order tests, manage chronic conditions, refer to specialists, and maintain documentation while collaborating with the care team.

Qualifications

• Master’s or Doctoral degree in Nursing.
• Current Illinois RN and APRN licensure.
• Certification as a Family Nurse Practitioner.
• Current CPR/BLS certification.
• Previous primary care or related clinical experience preferred.
• Strong communication, interpersonal, and patient care skills.
• Bilingual/Spanish preferred, but not required.

Responsibilities

• Provide primary care to patients of all ages, from infants to older adults.
• Conduct physical examinations and diagnose and treat common illnesses and injuries.
• Order and interpret diagnostic tests and prescribe medications as appropriate.
• Manage acute and chronic health conditions and provide preventive care and health education.
• Refer patients to specialists when necessary and collaborate with other healthcare providers to ensure coordinated care.
• Maintain accurate and timely patient documentation.
• Participate in quality improvement initiatives and support established clinical policies and practices.
