Job summary

Community Health Partnership of Illinois seeks a compassionate Family Nurse Practitioner to join our team in Aurora, Illinois. The FNP will provide comprehensive primary care across the lifespan, emphasizing accessible and culturally responsive healthcare.

The role requires a Master’s or Doctoral degree in Nursing, Illinois RN/APRN licensure, and FNP certification; CPR/BLS is also required. Bilingual Spanish is a plus.

Qualifications

• Master’s or doctoral degree in Nursing.
• Current Illinois RN and APRN licensure.
• Certification as a Family Nurse Practitioner.
• Current CPR/BLS certification.
• Experience in primary care or related clinical field preferred.
• Strong communication and patient care skills.
• Bilingual/Spanish preferred, but not required.

Responsibilities

• Provide primary care to patients of all ages, from infants to older adults.
• Conduct physical examinations and diagnose and treat common illnesses and injuries.
• Order and interpret diagnostic tests and prescribe medications as appropriate.
• Manage acute and chronic health conditions and provide preventive care and health education.
• Refer patients to specialists when necessary and collaborate with other healthcare providers to ensure coordinated care.
• Maintain accurate and timely patient documentation.
• Participate in quality improvement initiatives and support established clinical policies and practices.

Education

In this role, you will provide high-quality primary care to patients across the lifespan while supporting CHP’s commitment to accessible, culturally responsive healthcare.

Community Health Partnership of Illinois is a 501(c)(3) not-for-profit organization that provides primary and preventive healthcare services to uninsured and underserved communities throughout northern and central Illinois. CHP provides access to medical, dental, and behavioral health services, as well as connections to other social services.
