Job summary

Community Health Partnership of Illinois (CHP) seeks a compassionate Nurse Practitioner to provide comprehensive, patient-centered primary care across the lifespan in Champaign, Illinois.

You will diagnose and manage acute and chronic conditions, order tests, develop treatment plans, and prescribe medications within your scope while collaborating with physicians and staff to coordinate care. CHP emphasizes quality, culturally competent care and adherence to policies.

Qualifications

• Master’s or Doctoral NP degree from an accredited program.
• Illinois APRN license or eligibility.
• National NP certification.
• Illinois RN license as required.
• DEA registration or eligibility to obtain registration.
• BLS/CPR certification.
• Strong clinical assessment, diagnostic, and patient-care skills.
• Excellent communication, organizational, and interpersonal skills.
• Ability to work with diverse populations.
• Experience in primary care or FQHC environment preferred.
• Bilingual skills are a plus.

Responsibilities

• Provide comprehensive primary care services to patients across the lifespan.
• Conduct physical examinations, health assessments, and routine preventive care.
• Diagnose and manage acute and chronic health conditions.
• Order, perform, and interpret appropriate diagnostic and laboratory tests.
• Develop and implement individualized treatment and care plans.
• Prescribe medications and treatments within the scope of practice.
• Educate patients regarding diagnoses, medications, preventive care, and healthy lifestyle practices.
• Maintain accurate, timely, and complete documentation in the electronic health record (EHR).
• Collaborate with physicians, nurses, clinical staff, and other healthcare professionals to coordinate patient care.
• Make appropriate referrals to specialists and community resources when necessary.
• Follow CHP clinical policies, procedures, quality standards, and regulatory requirements.
• Participate in quality improvement and population health initiatives.
• Perform other related duties as assigned.
