Job summary

Community Health Partnership of Illinois (CHP) is a nonprofit healthcare organization delivering primary medical, dental, and behavioral health services to migrant workers and underserved populations across Illinois. We are seeking a compassionate RN to join our Champaign team, delivering patient-centered care and collaborating with providers to improve community health.

Responsibilities include patient assessment, vital signs, medication administration, and education.

Qualifications

• Current, active RN license in Illinois.
• Graduate of an accredited nursing program.
• Previous outpatient, primary care, community health, or other clinical experience preferred.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and patient-service skills.
• Ability to work both independently and collaboratively as part of an interdisciplinary healthcare team.
• Strong organizational skills and attention to detail.
• Commitment to providing respectful, culturally responsive care to patients from diverse backgrounds.

Responsibilities

• Assess patients and monitor changes in health status.
• Obtain and document vital signs, medical histories, and other clinical information.
• Provide nursing care according to established clinical protocols and provider direction.
• Administer medications, immunizations, and treatments as ordered.
• Assist providers with examinations, procedures, and follow-up care.
• Provide patient education regarding medications, treatment plans, preventive care, and chronic disease management.
• Coordinate patient care and communicate with providers and other members of the healthcare team.
• Respond appropriately to patient questions and clinical concerns.
• Maintain accurate, timely, and confidential documentation in the electronic health record.
• Support infection-control, quality, and patient-safety practices.
• Help connect patients with appropriate follow-up care and community resources.
• Maintain compliance with applicable clinical, organizational, and regulatory requirements.
