Job summary

Mount Carmel, a Mount Carmel Health partner, seeks a Nurse Practitioner in Primary Care to provide comprehensive care in Gahanna or New Albany. This full-time APP role emphasizes health promotion, disease prevention, and improving access to care in outpatient settings.

The APP works within a collaborating physician arrangement, leads care planning, and participates in performance improvement to drive high-quality outcomes for diverse patient populations.

Qualifications

• Masters required in Science, Nursing or specialty field.
• Graduate of Advanced Practitioner Program.
• Current License as RN and Nurse Practitioner in Ohio.
• Certified as Advance Practice Nurse in designated specialty.
• BLS provider certification and DEA license are required.

Responsibilities

• Manages the care and treatment of patients as indicated in the professional scope of practice and within current standards and protocols.
• Develops a comprehensive plan of care with the patient/family consistent with established guidelines.
• Practices within the guidelines of a Standard Care Arrangement with a collaborating physician.
• Initiates and leads performance improvement activities with a focus on achieving top quartile on clinical indicators.

Employment Type: Full time Shift: Description: Nurse Practitioner, Primary Care - Gahanna or New Albany

Position Purpose: This full‑time Advanced Practice Provider (APP) position supports the delivery of high‑quality, patient‑centered care in a Mount Carmel Gahanna and New Albany primary care office setting.

The APP provides comprehensive health services across the lifespan, with a strong emphasis on health promotion, disease prevention, and improving community access to care.

APPs at Mount Carmel are highly valued members of the care team and are offered excellent compensation, benefits, and an annual CME stipend.

Experience in the outpatient primary care setting is preferred.

Responsibilities

Minimum Requirements

• Strong clinical assessment and analytical skills.
• Self-directed professional with strong organizational skills.
• Ideally 2+ years of APP experience.
