A leading medical group in Brooklyn, New York is seeking a healthcare professional to provide patient assessments and coordinate care. Candidates must hold an active NYS APN license and DEA certification, with experience in primary care preferred. The role includes performing physical examinations, ordering diagnostic tests, and providing women's health services. Qualifications

• Previous experience in primary care preferred.
• Must be able to coordinate patient care and perform routine health maintenance.
• Knowledge of performing women’s health tasks including pap smears.
• Previous experience in primary care preferred.

Responsibilities

• Perform physical examinations and patient assessments.
• Order laboratory and diagnostic procedures as needed.
• Make recommendations for appropriate patient treatment plans.
• Make recommendations for appropriate patient treatment plans based on assessment.
• Initiates treatment by prescribing medications and ordering diagnostic tests and procedures.
• Reviews lab results and adjust plans according to results.
• Coordinate care and provide routine health maintenance for established and new patients.
• Provide preventive health services by counseling and educating patients and families.
• Provide women’s health care, including Pap smears and related tasks.
• Assist in quality management.

Essential Functions

• In collaboration with supervising physician, perform physical examinations and patient assessments to manage a range of patient health and wellness issues.
• Order laboratory and diagnostic procedures as needed.
• Obtain patient medical histories and document pertinent health history data and results of exam according to established standards.
• Make recommendations for appropriate patient treatment plans based on assessment of needs.
• Initiates treatment by prescribing medication, ordering diagnostic tests, exams, and procedures.
• Reviews lab results and propose plan of action according to results.
• Coordinate care and perform routine health maintenance for established and new patients.
• Provide preventive health services by counseling and educating patients and families.
• Provide care for women\'s health which includes performing pap smears and other related tasks.
• Assist in quality management.
