Job summary

Bluestone Physician Services is seeking a part-time Nurse Practitioner or Physician Assistant to provide on-site primary care within Assisted Living and Memory Care communities around Plymouth, MN.

You will manage a patient panel, design care plans, and collaborate with a team of CLINICAL Assistants, LPNs, RNs and social workers, with flexible 2.5 days per week.

This role offers competitive compensation in USD and supports professional collaboration and patient-centered care.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board certification or board-eligible through AANP/ANCC/NCCPA.
• Licensed or eligible for licensure by the stateBoard.
• DEA certification for the state of practice required/obtainable.
• Strong verbal communication and interpersonal skills.
• Excellent customer service and professional appearance.
• Valid driver's license and auto insurance; local travel required.
• Preferred: 2+ years clinical NP/PA experience.

Responsibilities

• Provide on-site, team-based primary care in Assisted Living and Memory Care.
• Care for patients Monday-Thursday, about 2.5 days weekly; days optional.
• Manage a panel of roughly 100 patients (avg 14/day).
• Design and manage individualized care plans for patients.
• Clinical support from LPNs, RNs and Social Workers.
• Access to labs and x-ray services as needed.
• Collaborate with home care, hospice and community partners.

Join our growing practice as a Part-time Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Plymouth, MN.

Position Overview

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Part-time providers care for patients Monday-Thursday approx. 2.5 days per week; days are optional for those working part-time
• Manage a panel of 100 patients (approximately 12-16 patients per/day with 14 being the avg.)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as appropriate

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
• Ability to obtain and /or maintain DEA certification for the State of required of work
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
