Qualifications

Job description

Nurse Practitioner / Physician Assistant - Primary Care

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and provide exceptional team‑based care to patients residing at Assisted Living and Memory Care communities in and around Minneapolis.

Position Overview

• Provide on‑site, team‑oriented primary care to patients within Assisted Living and Memory Care communities
• Full‑time providers care for patients Monday‑Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 200 patients (approximately 12‑16 patients per day with 14 being the average)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
• Access to diagnostics such as labs and x‑rays as needed
• Collaborate with community partners within home care, hospice, and other resources as appropriate

Qualifications

• Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA
• Licensed or ability to obtain licensure through the Minnesota Board of Nursing or Board of Medical Practice
• Ability to obtain and /or maintain DEA certification for the State of Minnesota
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid and unrestricted driver’s license and minimum state‑required liability auto insurance; local travel is required

Preferred Qualifications

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
