Job summary

Bluestone Physician Services invites a Nurse Practitioner (NP) or Physician Assistant (PA) to provide on-site, team-based primary care for residents in Assisted Living and Memory Care communities in and around Mankato, MN. Providers work full-time, Monday–Thursday, with an optional Friday for clinical or administrative duties, managing a ~200 patient panel with scheduling flexibility.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification as required by work state.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid and unrestricted driver's license; local travel is required.

Position Overview

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 200 patients (approximately 12-16 patients per day with 14 being the avg.)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as appropriate

Qualifications

• Ability to obtain and/or maintain DEA certification for the State of required of work
• Proven ability to communicate effectively with strong verbal skills

• Must have a valid and unrestricted driver's license and minimum state-required liability auto insurance; local travel is required

Preferred Qualifications

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
