Job summary

Bluestone Physician Services is seeking a Nurse Practitioner or Physician Assistant to deliver on-site primary care in Assisted Living and Memory Care communities around Burnsville. You will join a team delivering proactive, patient-centered care with coordination across clinicians.

The role offers full-time hours, a flexible schedule, and access to diagnostics, with strong emphasis on collaboration, care plans, and quality outcomes for complex patients.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification for the state of work.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid and unrestricted driver’s license; local travel is required.

Responsibilities

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
• Full-time providers care for patients Monday-Thursday; option for Fridays as clinical or administrative day.
• Manage a panel of about 200 patients with ~12–16 per day.
• Design and manage your schedule and care plans aligned with patient goals.
• Clinical support from CNAs, LPNs, RNs andSocial Workers.
• Access to labs and x‑ray diagnostics as needed; collaborate with home care, hospice, and other resources.

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Burnsville.

Position Overview:

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as appropriate

Qualifications:

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
• Ability to obtain and /or maintain DEA certification for the State of required of work
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required

Preferred Qualifications:

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
