Job summary

Bluestone Physician Services in Minnesota seeks a Nurse Practitioner or Physician Assistant to deliver on-site, team-based primary care within Assisted Living and Memory Care communities around Owatonna, MN.

Full-time schedule Mon–Thu with optional Friday for clinical or administrative duties. Manage a ~200-patient panel with support from LPNs, RNs and Social Workers; travel is local to communities in MN, WI, and FL.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Board Certified or Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or eligible for licensure in required state.
• Ability to obtain DEA certification as required by state.

Responsibilities

• Provide on-site, team-oriented primary care within Assisted Living and Memory Care communities.
• Care for a panel of ~200 patients (12–16 per day; avg ~14).
• Design and manage your schedule with Clinical Support from LPNs, RNs and Social Workers.
• Access to diagnostics (labs, x-rays) as needed.
• Collaborate with home care, hospice and other community resources.

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing at Assisted Living and Memory Care communities in and around Owatonna, MN.

Position Overview:

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day
• Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers
• Access to diagnostics such as labs and x-rays as needed
• Collaborate with community partners within home care, hospice, and other resources as appropriate

Qualifications:

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice
• Ability to obtain and /or maintain DEA certification for the State of required of work
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required

Preferred Qualifications:

• 2+ years of clinical experience as a Nurse Practitioner or Physician Assistant
