Position Overview

Stanford's Division of Nephrology is seeking an experienced Advanced Practice Provider (APP) to join our Outpatient Nephrology/Hypertension (HTN) Clinic team. The providers at Stanford's Division of Nephrology see patients across the bay area for general nephrology, as well as subspecialty needs like kidney transplant, glomerular disease, hypertension, stone, peritoneal and home hemodialysis. This APP position aims to help grow the HTN Center of Excellence within the division.

Key Responsibilities

• Provide comprehensive care to patients with nephrology and hypertension needs.
• Manage return patients independently in clinic and via telehealth.
• Work a 4-day week, 10-hour days, seeing up to 12 patients per day.
• Allocate time for indirect patient care after clinic hours.
• Collaborate with a multidisciplinary team to ensure high-quality patient care.

Qualifications

• Prior experience in Nephrology, Internal Medicine, or chronic disease management as an NP or PA is preferred.
• Strong clinical and communication skills.
• Commitment to patient-centered care and continuous improvement.
